Toby R Meltzer MD
Toby R Meltzer MD performs approximately 200 Gender Confirmation Surgery (GCS) cases per year. He performs both MtF and FtM genital reconstruction surgeries.
Dr. Meltzer works with a team for FtM GCS which includes both a Urologist and an OB/Gyn.
In addition, Dr. Meltzer performs Facial Feminization Surgery (FFS) to feminize the face and body as well as standard cosmetic procedures.
Toby R Meltzer MD graduated from LSU Medical School in 1983 and is board certified in both General and Plastic Surgery. An active member of the American Society of Plastic and Reconstructive Surgeons, he started practicing at Oregon Health Sciences University in 1990 and began private practice in 1996 in Portland, Oregon.
In 2003 he moved to Scottsdale, Arizona to expand his practice with privileges at Scottsdale Healthcare Osborn Campus at the Greenbaum Surgery Center and Scottsdale Healthcare Shea Campus.
In 2019 the Meltzer Clinic expanded and we also began offering our services at Legacy Good Samaritan Medical Center in Portland, Oregon.
Ellie Zara Ley MD
Ellie Zara Ley MD is an accomplished board certified plastic surgeon. Her impressive surgical training background, with multiple plastic surgery subspecialty fellowships allow her to provide superb comprehensive care to the transgender community. After completion of her training, Dr Ley became an assistant clinical instructor at the University of Utah. She enjoyed teaching the full breadth of plastic surgery to the new generation of young surgeons. She then moved her practice to serve southern Arizona before coming to Scottsdale to join Dr. Meltzer along with his wonderful staff to specialize in plastic and transgender surgery.

Nick Esmonde MD
The Meltzer Clinic is excited to introduce, Nick Esmonde M.D. to our practice. Dr. Esmonde comes to our practice from Oregon Health Science University in Portland, Oregon; where he recently finished his plastic surgery residency. While Dr. Esmonde is new to our office, he has spent the last several years working in the Gender Surgery program at OHSU and has extensive knowledge in the area. Dr. Esmonde will spend the remainder of the year training with Dr. Meltzer and Dr. Ley in Scottsdale, AZ and Portland, OR. He will serve as our full-time Associate Surgeon in Portland starting June 2021!
Dr. Meltzer follows WPATH guidelines for surgery:
- You will need to provide two referrals from qualified mental health professionals for genital procedures; one referral for other procedures unless otherwise specified by insurance.
- A letter from your hormone specialist stating you have been on 12 months of continuous hormones for genital procedures, unless clinically contraindicated.
- Dr. Meltzer will operate on minors with parental consent.
